There are two ways to sign up for Dr. Jason Mazzarella, DC seminars.

1.  The first step is to fill out the boxes below. Provide your name, email address and indicate which seminar you would like to attend.
2. The second step to complete registration is to pay for the seminar. With this step there are two options.
3. Select the PayPal credit card button on the right of the screen and pay through your PayPal account.
4. In the Message box below that you would like to receive an Email Invoice and then Submit a Cheque (Check). The Cheque (Check) should be made out to North American Spine Institute.  

Please note that you are NOT registered for the course until an Email conformation is sent to you. If you do not receive an email conformation within 24-48 hours please contact our office at 647-991-7246.
